Uneven or Asymmetric Lips in London: How a Consultant Dermatologist Assesses and Corrects Lip Asymmetry with Filler

Most people first notice lip asymmetry in photographs. One side of the upper lip sits slightly higher, one peak of the Cupid’s bow is less defined than the other, or the lower lip looks fuller on one side when you smile. For some patients the difference has always been there. For others it appeared after dental work, an injury, or a previous filler treatment that settled unevenly. At Dermasurge Clinic, Dr Hiba Injibar, a consultant dermatologist on the GMC specialist register, starts by assessing why the lips are uneven. Only then does she consider whether filler is the right tool, because a good deal of lip asymmetry has little to do with volume.

What is lip asymmetry?
No face is perfectly symmetrical, and small differences between the left and right sides of the lips are normal. Asymmetry becomes a clinical question when the difference is noticeable at conversational distance, when it bothers the patient, or when it has changed recently. Each of those points to a different conversation.
Uneven lips can involve the volume of the lip tissue, the shape of the vermilion border (the line where lip meets skin), the position of the corners of the mouth, or the way the lips move when you speak and smile. A patient may describe all of these as “uneven lips”. Treating them well depends on telling them apart.
Common causes of uneven lips
- Developmental differences: natural variation in lip volume or border definition, often present since adolescence.
- Dental and jaw factors: tooth position, crowding, missing teeth, orthodontic changes, bite alignment and underlying jaw asymmetry all change how the lips sit.
- Muscle pull: uneven activity in the muscles around the mouth, including after facial nerve injury or Bell’s palsy, can make one side lift or drop.
- Trauma and scarring: cuts, sports injuries, previous surgery or cleft lip repair can alter tissue and contour.
- Previous filler: product that has migrated, been placed unevenly, or built up over repeated treatments.
A new lump, a patch that will not heal, or a change in colour or texture on the lip needs a medical assessment before anything cosmetic is considered. The lips are exposed to the sun, and sun-related changes such as actinic cheilitis are diagnosed and managed as a dermatological condition first.

Who is lip asymmetry treatment suitable for?
Lip filler is only available to adults. UK law prohibits cosmetic injectables for anyone under 18, and Dermasurge Clinic does not treat under-18s for cosmetic lip concerns.
Suitable patients are generally in good health, have a stable lip shape (not in the middle of orthodontic treatment, for example), and want a more balanced appearance without expecting perfect symmetry. Treatment is usually postponed during pregnancy or breastfeeding, during an active cold sore or infection near the mouth, and shortly after significant dental work. Some autoimmune conditions and medications also need careful review.
Part of the consultation is understanding how the asymmetry affects you. When the level of concern seems out of proportion to what is visible, Dr Injibar will discuss this openly, because more filler is rarely the answer in that situation.

How does a consultant dermatologist assess lip asymmetry?
The assessment is structured, and it takes longer than many patients expect. That time sets realistic expectations, and it is where the most important decisions are made.
Structural versus volume asymmetry
Dr Injibar first looks at the foundations. Teeth, gums and jaw support the lips from behind, so a tooth that sits further forward on one side will push the lip forward on that side. A jaw that is slightly rotated will tilt the whole mouth. These are structural causes. Filler can sometimes soften how they look, but it cannot correct them.
Volume asymmetry is different. One side of the lip has more or less tissue, or the border is less defined on one side. This is the type of asymmetry filler is best suited to, and it is often the smaller share of the overall picture.
Watching the lips move
Lips that look even at rest can become lopsided when you smile, speak or purse them. Dr Injibar watches the mouth in motion to see whether one side is pulled more strongly by the surrounding muscles. Muscle-driven asymmetry behaves differently from volume loss, and adding filler to the weaker side can make movement look heavier or less natural.
Reviewing previous filler history
If you have had lip filler before, Dr Injibar will ask when, what product was used if known, how much, and how the lips looked before and after. She will also examine the lips by touch, feeling for firm areas, nodules, or product that has moved beyond the lip border. This history often explains asymmetry that appeared gradually over several treatments.
Assessment with a consultant dermatologist is required to determine the appropriate treatment.
Why filler cannot correct every cause of uneven lips
Filler adds volume. That is its strength and its limit. If one lip sits higher because of tooth position, adding filler to the lower side makes both lips bigger without addressing the cause, and the result can look heavy. If one corner of the mouth drops because of nerve weakness, filler will not restore that movement.
Where the main driver is dental or skeletal, Dr Injibar may suggest discussing it with a dentist, orthodontist or maxillofacial specialist before any cosmetic work. Where muscle activity is the main factor, other approaches may be discussed at consultation. Some of these involve prescription-only medicines, which can only be considered after a face-to-face medical assessment.
In many cases the most useful outcome of the consultation is a clear explanation of what is causing the asymmetry. Sometimes that means advising against filler altogether. Sometimes it means a smaller, more targeted treatment than the patient expected.
When should old filler be dissolved first?
Correcting uneven lip filler is one of the most common reasons patients come to Dermasurge with this concern. If the unevenness comes from previous hyaluronic acid filler, adding more product on top usually makes the problem worse. The lips can end up overfilled, the border can blur further, and the asymmetry often remains.
Dr Injibar may recommend dissolving existing filler before any correction when she finds:
- filler that has migrated above the lip border, sometimes called a “filler moustache”
- lumps, nodules or uneven firmness when the lip is examined
- product that has built up over several treatments and now looks disproportionate
- a lip shape that has drifted from its natural anatomy
Hyaluronic acid filler is dissolved with a prescription-only enzyme, used only after assessment. It is not suitable for everyone, and allergy history is checked beforehand. Dissolving only works on hyaluronic acid products, so knowing what was used before matters. After dissolving, the lips are given time to settle, typically at least two weeks, before they are reassessed. Some patients find that once the old product has gone, their natural lips are more balanced than they expected and need very little further treatment.
Not every patient who has had filler before needs it dissolved. When the previous product has integrated well and the asymmetry is small, a targeted correction may be appropriate. Dr Injibar makes this decision clinically and does not follow a fixed rule.

Correcting lip asymmetry with filler at Dermasurge
When volume asymmetry is the main issue and filler is appropriate, treatment is planned to restore balance rather than add size. You can read more about the clinic’s approach on the dermal fillers treatment page.
Planning the correction
Asymmetrical lips filler treatment usually uses small volumes placed precisely. That might mean treating only one side, defining a single section of the border, or supporting one corner of the mouth. Dr Injibar maps the lips at rest and in movement, and agrees the plan with you before any product is placed. The aim is lips that look like your own, with the imbalance reduced.
The treatment appointment
A topical anaesthetic is applied first, and most hyaluronic acid lip fillers also contain local anaesthetic. The lips have a dense blood supply, and a detailed knowledge of the vascular anatomy is central to safe injecting. Dr Injibar uses a needle or cannula depending on the area and the depth required. Where the correction is complex, she may deliberately under-correct at the first appointment so the lips can be reviewed once they have settled.
Recovery and review
Swelling is common for 48 to 72 hours and can temporarily make the lips look more uneven than before, which many patients find unsettling. Bruising is possible. A review appointment at around two weeks lets Dr Injibar assess the settled result and make a small adjustment if needed. Results can last up to six to twelve months, depending on the product, the area treated and individual factors. Outcomes vary between patients, and no specific result can be guaranteed.
Why choose Dermasurge Clinic for lip balance in London
Lip asymmetry sits between medical and cosmetic dermatology. The Dermasurge Clinic model is designed for that overlap.
- Consultant-led care: every assessment is carried out by Dr Hiba Injibar, a consultant dermatologist on the GMC specialist register.
- Medical and cosmetic dermatology under one roof: any lesion, scar or skin change on the lip is assessed medically before cosmetic treatment is discussed.
- On-site laser platforms: M22, Q-switched and IPL systems are available for related concerns such as scarring, redness or pigmentation around the mouth. Laser suitability depends on skin type (Fitzpatrick), requires strict sun avoidance and daily SPF before and after treatment, and any pigmented lesion is assessed first to rule out anything suspicious.
- Evidence-based, personalised treatment plans: recommendations are based on clinical findings, including advice not to treat where filler would not help.
If you would like to understand what is behind your lip asymmetry, arrange an assessment with a consultant dermatologist. Assessment is required before any treatment is recommended.
Frequently asked questions
Is it normal for lips to be slightly uneven?
Yes. Most people have some difference between the left and right sides of their lips. Treatment is only considered when the asymmetry is noticeable, bothers the patient, and has a cause that filler or another approach can address. A recent change in lip shape, or any new lump or patch, should be assessed medically first.
Can filler correct lips that became uneven after a previous filler treatment?
Often, but not always by adding more filler. Where previous hyaluronic acid filler has migrated, formed lumps or built up over time, dissolving it before any correction usually gives a more natural and predictable result. Dr Injibar decides this after examining the lips and reviewing your treatment history.
Will I definitely need my old filler dissolved?
No. Dissolving is recommended when existing product is contributing to the asymmetry or has moved outside the lip. If previous filler has integrated well and the imbalance is small, a targeted correction may be possible without dissolving. This can only be decided at an in-person consultation.
How long does lip asymmetry correction with filler last?
Results can last up to six to twelve months. This varies with the product used, the area treated and individual factors such as metabolism. Because asymmetry corrections often use small volumes, some patients find they need less maintenance than with general lip treatment. Results vary, and a review at around two weeks is part of the plan.
Can lip filler fix asymmetry caused by my teeth or jaw?
Filler can sometimes soften the appearance of structural asymmetry, but it cannot correct the underlying cause. Where tooth position or jaw shape is the main factor, Dr Injibar may suggest a dental or orthodontic opinion first, so that any cosmetic treatment is planned around a stable foundation.
Uneven lips have many causes, and the right approach depends on an accurate assessment. Assessment with a consultant dermatologist is required to determine the appropriate treatment.
